WeCare Centers is hiring a Regional Discharge Coordinator who is responsible for coordinating and managing the resident’s transition through the continuum of care from admission to discharge and post-discharge follow-up. This role ensures that all discharge planning services are resident-centered, compliant with federal and state regulations, support quality outcomes, maximize benefits and resources, and promote safety and continuity of care. The Regional Discharge Coordinator collaborates regularly with a wide range of interdisciplinary partners, residents, family, community partners and more.
This role is a hybrid position with the opportunity to work remotely, with travel to our communities throughout Western PA as needed on a regular basis.

Key Responsibilities
- Admission and Care Transition Coordination
- Care Planning and Ongoing Case Management
- Discharge Planning and Execution
- Post-Discharge Follow-Up
- Regulatory Compliance and Documentation
- Quality Assurance and Performance Improvement (QAPI)
- Travel as required to any of our communities throughout Western PA
Skills, Knowledge and Expertise
- Degree in Social Work, Nursing, Administration, Case management or related LTC field preferred.
- Experience in discharge planning or care management in a Skilled Nursing Facility
- Knowledge of Medicare/Medicaid, managed care plans, PDPM concepts, and community resources preferred.
- Working knowledge of CMS State Operations Manual, Appendix PP, and discharge-related F-Tags (F622–F626).
- Strong communication, organizational, motivational interviewing, and problem-solving skills.
