Job Overview

AleraCare has a unique Pharmacist opportunity for those looking to shape the future of pharmacy care in a unique setting. AleraCare operates nearly 40 Ambulatory Infusion Clinic locations and supports them through a centralized order processing model. The pharmacist role is critical to ensuring accuracy, safety and appropriateness of all orders (through order validation) as well as ensuring the needed clinical elements are supplied to various payer utilization management programs so that patients can begin and continue with their critical infusion therapy. The pharmacist works collaboratively to problem solve for complex therapy orders. The pharmacist is also involved in creating educational tools, trainings and content for both non-clinical and clinical (nurses) staff. There is opportunity to execute first in setting clinical programs and influence their content as well as ensure optimal execution. This role is critical to clinical decision making for the organization, influencing growth and program opportunities. Communication with physicians, patients and internal clinical and non-clinical business associates occurs regularly.

 

 

This is a Monday through Friday position.

Day to Day Primary Functions:

  • Supports intake team questions and provides clinical support to patients as needed.
  • Performs quality checks on new orders ensuring safety, appropriateness and effectiveness on infused/injectable medications.
  • Perform patient drug counseling.
  • Executes on all clinical programs within the organization.
  • Contributes to the design, development and implementation of new clinical programs.
  • Communicates needed updates to order/referral forms.
  • Performs system testing for code releases (clinical pieces).
  • Assists with clinical program reporting and performance.
  • Manage PA denials through appeals process.
  • Calls patients to both educate and address clinical concerns.
  • Oversee intake referrals/prescriptions, validates for accuracy and provides referral entry support.
  • Performs job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations.
  • Consistently works (multi-tasks) and problem solves independently and effectively with minimal supervision.
  • Takes charge when necessary and is looked to for direction. Makes the right decisions and uses good judgement.
  • Is cross trained and multifunctional maintaining competency in multiple areas.
  • Enforces the formulary system and follows departmental policy and procedure when acquisition of non-formulary product is judged necessary.
  • Participates in cost effective use and selection of pharmaceuticals.
  • Supports and fields nursing questions regarding drugs.


Qualifications

  • Minimum of 2 years actively practicing as a licensed pharmacist
  • RPH or Pharm D
  • Experience working in managed care settings (ie PBM) preferred
  • Experience interacting with physicians and patients telephonically
  • Highly organized
  • Exceptional communication skills
  • Detail oriented with the ability to also be strategic
  • Outstanding computer literacy and operational function skills
