Role Overview
As a Patient Care Coordinator, you will serve as the primary point of contact for patients, managing appointment schedules, care updates, and communication with the internal care team. This role requires strong organization, empathy, and precision to ensure patients receive high-quality, timely support.
Your Impact
You will help patients navigate their care journey with confidence, contributing to improved satisfaction, reduced no-shows, and smoother coordination across medical providers.
Core Responsibilities
Care Navigation – 50%
- Act as the main liaison between patients and the care team, ensuring clear, supportive communication
- Schedule and manage patient appointments and follow-up visits
- Send reminders and confirm patient attendance to reduce no-shows
Communication & Support – 30%
- Provide clear guidance and updates to patients regarding next steps in their care plan
- Answer patient questions with empathy and professionalism
- Coordinate with healthcare staff to ensure timely responses and actions
Records & Compliance – 20%
- Maintain accurate and updated patient information in EMR systems
- Ensure all processes follow HIPAA standards and healthcare compliance
- Document all interactions clearly for team visibility
Requirements
Must-Haves
- 1+ years of experience in patient coordination, patient navigation, or healthcare support
- Strong English communication skills (verbal and written)
- Experience working with EMR/EHR systems
- High attention to detail and strong organizational skills
- Ability to manage high-volume communication in a fast-paced environment
Nice-to-Haves
- Experience working with U.S. healthcare or clinical environments
- Background in call center, patient support, or telehealth
- Familiarity with HIPAA or healthcare compliance
Tools Proficiency
Required
- EMR/EHR systems (Athena, DrChrono, Kareo, or similar)
- Communication Tools: Gmail, Zoom, Slack
- Documentation & Workflow: Google Workspace (Docs, Sheets, Drive)
- Scheduling: Calendly or internal appointment systems
Preferred
- Customer Support Tools: Zendesk, Freshdesk, or similar
- Project Management: Notion, Asana, or ClickUp
