Social Worker

The Social Worker supports participants and their families with counseling, care coordination, and resource connection. They conduct assessments, assist with transitions, mediate family concerns, and help ensure the psychosocial well-being of all participants. This role requires empathy, professionalism, and strong communication skills to navigate sensitive situations and promote mental wellness.

   Conduct psychosocial assessments and develop service plans.

·      Provide counseling, crisis intervention, and support for participants and families.

·      Coordinate community resources, benefits, and referrals.

·      Maintain detailed records and progress notes.

·      Collaborate with healthcare and activities staff to promote emotional wellness.

 

 Bachelor’s or master’s degree in social work and or clinical Psychology (MSW preferred).

·      Current Maryland social work license (LBSW or LCSW-C).

·      Experience in gerontology, mental health, or community services.

·      Excellent communication and empathy skills.

·      Ability to lift, push, and/or pull up to 50 lbs.

·      Ability to stand for extended periods
