About the Service

Together is working to provide this new service, delivering support for people age 18+ with an SMI (Severe Mental illness) for their Annual Physical Health Check (PHC), across Norfolk & Waveney.

The project is looking to promote and support Norfolk and Waveney workforce to build understanding about the physical health risks facing people living with an SMI, increased likelihood of key health conditions and reduced life expectancy.

You will also support people with an SMI to engage with their health check and interventions and/or lifestyle changes which will reduce the health inequalities noted for this population group.

Key Responsibilities

You will help promoting the importance of Annual Physical Health Check for SMI clients by making telephone/virtual contact with as many people as possible to offer support to book their SMI Physical Health Check and provide support to make arrangements for SMI Physical Health Check. This will also include data input and liaising with clinical professionals at the GP Practices as well as other mental health providers.

You will also arrange a visit to clients’ home, care home or supported living facilities to perform Physical Health Check on behalf of the GP practices in Norfolk & Waveney.

You will also help promoting the project in the community through various events in Norfolk & Waveney area with information stall and perform health check elements if appropriate.


Skills, Knowledge & Expertise

We are looking for highly motivated, self-starting and imaginative individuals with great leadership and collaboration skills, with a can-do attitude and the motivation to change and enhance the service during this exciting period.

Our ideal candidate will have excellent communication skills and experience of supporting or interacting with service users with complex needs, working with clinical colleagues and external agencies and stakeholders.
