Position Summary:
AleraCare, a provider of infusion treatments and specialty pharmacy services, is seeking an experienced Nurse Practitioner (NP) to support patient care delivery in our Idaho and New Mexico infusion clinic locations. NP must be available to be onsite at one of the ID or NM clinics. 3 years of nursing experience required. Infusion experience is a plus. The NP assumes a pivotal role in managing clinical activities, supporting staff, and ensuring the safe, compliant, and effective delivery of infusion therapies in the clinic setting. We currently have 30+ ambulatory infusion centers in six western states. Our centers are open Monday – Friday, 8:30am-5:00pm. Job Overview
The Nurse Practitioner (NP) is responsible for providing direct patient care and clinical oversight in the outpatient clinic environment. This role involves managing patient care, supervising clinical and nonclinical staff, and ensuring high standards of practice in procedures such as infusions, injections, dressing changes, and other hands-on treatments. The NP will work within a designated clinic location and, in accordance with federal guidelines, remotely supervise other operating clinics via video and/or phone. The NP will focus on guiding and coordinating the work of their team members, ensuring clinic operations are completed safely and efficiently, and company and patient care goals are met. Responsibilities include but are not limited to:
Clinical
- Conducts comprehensive assessments, diagnoses conditions, and develops treatment plans for patients of all ages within scope of practice. This includes in-person office and telehealth visits.
- Performs and/or oversees procedures, including but not limited to:
- Infusions (i.e. IV medications, hydration therapy)
- Injections (i.e. medications, vaccines, biologics)
- Dressing changes
- Point of Care testing (POCT)
- Monitors patient progress and adjusts care plans as needed.
- Writes or modifies orders based on patient findings; interprets tests, treatments, and therapies within scope of practice.
- Prescribes medications and non-pharmacologic treatment modalities, as indicated
- Demonstrates competency and excellence in all procedures and skills considered essential in their area of practice.
- Ensures physical and documentation compliance with all company and clinic standards, assists with wayfinding, and follows up with open-ended tasks to overcome barriers of full compliance.
- Assists in developing clinical protocols, policies and procedures pertaining to professional practice and development.
- Collaborates with physicians and other healthcare providers to coordinate care.
- Travels or delegates travel to offsite locations as necessary.
Supervisory
- Acts as a clinical leader and point person for team members within an assigned region.
- Provides clinical and nonclinical oversight to Registered Nurses (RNs), Licensed Practical Nurses (LPNs) and nonclinical staff.
- Implements, supervises, and ensures adherence to clinical protocols, safety standards, and regulatory requirements.
- Participates in hiring and onboarding staff. Supports staff development through training, mentoring, and performance feedback; tracks and ensures compliance with training and annual competency requirements.
- Performs annual performance reviews for assigned region; assists with performance improvement plans as necessary.
- Ensures appropriate staffing for assigned region; finds staffing coverage for unplanned absences.
- Performs and maintains clinical oversight of all patient functions.
- Coordinates and conducts regularly scheduled (i.e. monthly) team meetings and actively participates in educational opportunities.
- Acts as a liaison between team and departmental leadership, facilitating problem-solving and communication.
- Keeps all lines of communication highly professional and works as a change agent to refocus others from unproductive activities; deescalates individual and team issues or unprofessional conduct.
- Assists with the implementation of strategic and operational plans to ensure adequate and appropriate staffing, facilities, and space. Upholds confidential information in accordance with company standards.
- Accepts change gracefully, assists in development, and leads to the implementation of company goals by collaborating with key stakeholders and team members.
- Aligns assigned regional team goals with AleraCare’s initiatives. Partners with Leadership to ensure fiduciary obligations are met and proper fiscal management is in place with a focus on quality improvement and cost effectiveness.
- Demonstrates accountability for own professional practice and improvement efforts, which focus on patient care delivery and support processes that provide optimal patient outcomes.
Leadership
- Works closely with their team members, providing guidance, support, and mentorship.
- Provides leadership, direction, administrative support, coaching, and feedback to the clinical and nonclinical staff within assigned region.
- Acts as a role model and enforces behavioral standards for service excellence. Monitors the overall execution of service excellence tactics with limited decision-making authority, in order to appropriately delegate to departmental leadership for final decision-making.
- Facilitates and emulates the role of the NP in solutions and resolution of issues related to patient care delivery and collaboration with all members of the health care team including interdisciplinary staff, patients, and their families.
- Maintains open lines of communication between departmental leadership is an agent for positive change.
Professional
- Works collaboratively with all members of the healthcare team to meet the needs of the company and department.
- Advocates for clinical practice at the extent of the team’s training and licensure; identifies and partners to remove barriers to clinical practice; acts as an advocate for patients, families, coworkers, and the profession of nursing.
- Maintains board certification, state licensure, and continuing medical education requirements as outlined by company, state, and federal policy.
Additional Responsibilities
- Establishes and maintains positive clinical rapport with all patients and their families, representing AleraCare including working directly or indirectly with patients and their families.
- Assists in securing and maintaining all licensure and accreditations warranted by AleraCare, including ACHC, Joint Commission, and any other licensure or credentialing as necessary.
- Ensures compliance and adherence to all State and Federal regulations and laws, OSHA, HIPAA, CPR, accrediting agency standards, all pharmacy and nursing licensure standards of practice.
- May be required to participate in afterhours on call responsibilities or to work any day or shift as required to accommodate patient schedules in administering medications.
- May be required to lift or carry equipment and supplies.
Qualifications
- Current and unrestricted Nurse Practitioner license in the state(s) of practice.
- Master’s or Doctorate degree in Nursing from an accredited program.
- Certification in Family, Adult-Gerontology, Acute Care NP, or equivalent.
- Minimum of 3 years of clinical experience, with at least 1 year in a supervisory or leadership role.
- Proficiency in outpatient procedures and chronic disease management.
- Strong communication, leadership, and organizational skills.
- Must be willing to travel to multiple clinics in assigned areas.
- Must have and maintain current CPR certification.
- Must have prior experience with electronic health records (EHR) and telehealth systems.
- Must have advanced computer and excellent customer service skills – written and verbal.
- Must be comfortable with onsite and remote supervision.
