Job Overview

The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Registered Nurse (RN) in Recuperative Care plays a pivotal role in delivering high-quality, compassionate healthcare to individuals experiencing homelessness. This position requires clinical expertise within an integrated, trauma-informed care model, with a focus on patients recovering from illness or injury. The RN is crucial in fostering patient-centered care and enhancing overall well-being through adaptability, resilience, and a proactive approach to addressing healthcare barriers. The Recuperative Care program is a unique program supporting individuals who have been discharged from the hospital, who are too frail or complex to properly heal on the streets or in a shelter. Patients stay in the program and have access to clinical staff 24/7 while they are recuperating from their hospital stay. The program comprises multiple disciplines working together to deliver holistic, integrated care. Roles within the care team include nurses, case managers, peer support specialists, medical assistants, psych providers, behavioral health clinicians, and primary care providers. The team functions as a cohesive and supportive partner with the patients. There is an intentional focus on medication management, education, care management, and navigation of resources.

Qualifications Summary

  •  A degree in nursing (BSN or MSN) from an accredited college or university is required.
  • Experience in ambulatory, public, community, mental health, emergency, or critical care nursing is preferred. 2 years minimum preferred.  
  • Pediatric nursing experience is highly desirable.
  • Experience or familiarity with serving diverse or marginalized communities, including those experiencing homelessness, is preferred.
  • Bilingual (English/Spanish) preferred.

Essential Job Functions

  • Provides nursing care to program participants at Stout Street Recuperative Care Center to accepted community standards of quality healthcare.
  • Works with local hospitals, community clinics, social service agencies, and other CCH programs to ensure appropriate continuity of care and to support referrals to the program.
  • Triages and supports program referrals based on patient acuity, program requirements, and bed availability.
  • Conducts rapid and accurate triage assessments both in-person and via telephone, providing appropriate care recommendations according to nursing protocols.
  • Administers medications safely and effectively according to protocols.
  • Educates and empowers patients by providing appropriate health education and information, addressing common chronic and acute health conditions such as diabetes management, hypertension management, nutrition, wound care, infectious disease, gender-affirming care, and family planning.
  • Oversees care for patients with transitional, chronic, and complex health conditions.
  • Mentor and serve as a preceptor to new nurses and nursing students, enhancing their clinical skills and confidence.
  • Facilitates access to internal programs such as health outreach and housing support services.
  • Participates in team-based or interdisciplinary quality improvement initiatives.
  • Attends required disciplinary, interdisciplinary, suite, quality improvement, and all-staff meetings. Ensures timely and accurate documentation of all patient interactions, clinical services, and communications in the electronic health record (EHR).
  • Performs other duties as assigned.
