How will you fight for an AIDS-free generation?
Reporting to the Clinical Doctor - Site Project Coordinator, the Nurse Mentor is responsible for the provision of integrated, comprehensive and high-quality HIV care and treatment services in designated health facilities. He/She will also provide mentorship to nurses and PSA of the HIV clinic by acting as a role model, staying up-to-date on current policies and procedures, providing advice for difficult situations, offering support to increase successful goals and providing clinical education.

Key Responsibilities and Accountabilities
Under the supervision of the Site Project Coordinator, he/she is responsible for:
- Ensure the diagnosis, care and treatment of people living with HIV and other co-infections such as tuberculosis, in accordance with the standards of the national HIV control program;
- Ensure that all PLHIV receive testing for index cases and that all sexual and biological contacts are linked to HIV testing services.
- Ensure that all PLHIV received are screened for chronic non-communicable diseases.
- Ensure that all eligible patients are enrolled in the various differentiated models of care offered by the health facility.
- Ensure that the monitoring of cohorts of PLHIV on ART is properly carried out according to national guidelines and that they are maintained on ART for as long as possible.
- Ensure the day-to-day monitoring of patients' clinical conditions throughout the continuum of care and document it in the appropriate records and tools and improve the archiving of clinical records.
- Organize with the APS the tracking of clients who missed their appointments or who have been lost to follow-up, including home visits;
- Assess patients with psychosocial and mental health problems and refer them to the competent services as needed (psychosocial counsellors, social services, psychology, psychiatry);
- Ensure the establishment of support groups for children, adolescents, and adults, and participate in facilitating these support groups.
- Ensure that all monitoring registers for PLHIV are properly filled out and up to date
- Ensure that all pregnant and lactating women received in the health facility receive HIV testing services, and that those living with HIV are put on ARVs;
- Ensure the clinical follow-up of HIV-exposed infants until they are discharged from the PMTCT cohort at 18 months by ensuring that they receive ARV and Cotrimoxazole prophylaxis and that they have benefited from the molecular diagnosis of HIV according to the national algorithm.
- Promptly identify and actively monitor pregnant and lactating women living with HIV and/or their exposed children who are lost to follow-up.
- Ensure that the cohort monitoring registers are properly completed and up to date.
- Oversee PMTCT community activities, including mapping pregnant women and their linkage to the health facility for care
- In collaboration with the other members of the clinical team, use standard nursing procedures to ensure that all patients benefit from care services in accordance with their uniqueness, specificity, values, skills and qualifications.
- Submit regular activity reports to the line manager
- Any other task assigned by the supervisor
