We are seeking a candidate who is based in the Boston MA.

*This role requires frequent travel; you're expected to be driving for several hours each day, conducting 3-5 home visits within your area.

Key Responsibilities :

• Conduct home visits to assess and create personalized care plans.

• Train and support caregivers, ensuring they can meet their loved ones' needs.

• Monitor and report on clients’ health, coordinating with their primary care physicians.

• Facilitate smooth transitions and ongoing care for clients.

• Maintain compliance with all relevant regulations and standards.

• Full-time Hybrid position offering autonomy while utilizing your nursing assessment, care planning and case management skill
