Job Overview

What You Will Do as a Community Liaison

  • Develop and maintain relationships with physicians, hospitals, discharge planners, skilled nursing facilities, assisted living communities, rehabilitation centers, faith communities, and other referral partners.
  • Manage assigned outreach territories and referral relationships through regular visits, communication, and account management activities.
  • Educate healthcare professionals, patients, families, and community members about hospice, palliative care, home health, and supportive services.
  • Coordinate and participate in educational programs, health fairs, community events, senior expos, networking meetings, and outreach initiatives.
  • Serve as a visible and professional representative of Good Samaritan throughout the community.
  • Build partnerships with community organizations, social service agencies, civic groups, educational institutions, and faith-based organizations.
  • Support patients and families by providing information, education, and guidance regarding available care options and services.
  • Collaborate with admissions, clinical, bereavement, volunteer, and leadership teams to support continuity of care and community engagement efforts.
  • Analyze referral trends and community needs to identify opportunities for growth and expanded outreach.
  • Maintain accurate documentation of outreach activities, referral interactions, and community engagement efforts.
  • Assist with organizational marketing initiatives, educational materials, newsletters, social media efforts, and community awareness campaigns.
  • Support mission-focused community programs and special events that strengthen Good Samaritan's presence throughout the region.

What We Are Looking For

  • Minimum of 2 years of experience in healthcare outreach, community relations, marketing, customer service, business development, or a related field preferred
  • Strong relationship-building, networking, and interpersonal communication skills
  • Excellent presentation, public speaking, and written communication abilities
  • Ability to work independently while managing multiple priorities and deadlines
  • Experience coordinating events, educational programs, or community outreach activities preferred
  • Knowledge of healthcare referral processes and community healthcare resources preferred
  • Proficiency with Microsoft Office and CRM/documentation systems
  • Creative, self-motivated, and highly collaborative approach to work
  • Ability to work flexible hours, including occasional evenings and weekends
  • Valid Virginia driver's license, reliable transportation, and acceptable driving record

Schedule & Benefits

Schedule: Full-time position with a typical Monday–Friday (8:30 am- 5 pm) schedule. Flexibility is required to attend community events, educational programs, networking activities, and occasional evening or weekend engagements.
