Position Overview
As a Nurse Practitioner, you will provide comprehensive primary care services to a defined patient population within an employer-sponsored clinic. Your work will focus on preventive care, chronic condition management, and addressing acute health needs — all within a predictable, weekday schedule.
This role offers a non-traditional care environment that prioritizes continuity, trust, and quality of care.
Schedule
- Monday, 10am - 6pm
- Wednesday, 10am – 6pm
- Friday, 10am - 6pm
- Start and end times are flexible

What You’ll Do
- Provide advanced clinical support including acute and episodic care, primary preventive care, and chronic disease management
- Collaborate with HPP nurses on claims analysis
- Establish clinical pathways and treatment protocols
- Conduct telehealth visits as needed
- Coordinate care with patients’ external provider networks
- Act as a clinical consultant while upholding exceptional standards of care
- Maintain required licensure and certifications
What You Bring
Required Qualifications
- Master’s degree in Nursing (from an accredited program)
- Active, unrestricted Michigan RN license
- Active, unrestricted FNP certification (AANP or ANCC)
- Active Michigan NP Specialty Certification
- 2+ years of nursing experience (med-surg, ER, ICU, acute care, public health, school nursing, or outpatient preferred)
- BLS certification (AHA or Red Cross) within 90 days of hire
Preferred Qualifications
- DOT Examiner Certification (or willingness to train)
- Additional specialty certifications encouraged (Health Coaching, Case Management, Primary Care, Emergency/Hospital-based specialties, Women’s/Men’s Health, Sports Medicine)
