The Clinical Associate (LVN/LPN) is an integral member of the Patient Aligned Care Team (PACT) at a VA Community Based Outpatient Clinic. Working under the supervision of a Registered Nurse and licensed provider, the LVN/LPN supports safe, efficient, and Veteran-centered primary care through rooming, clinical procedures, medication administration within scope, health coaching, and care coordination. The role emphasizes access, quality, safety, and whole health in alignment with VA directives and evidence-based practice.

  • Greet and room Veterans; obtain and document vitals, pain, allergies, medication history, and targeted histories using clinical reminders.
  • Perform point-of-care testing (e.g., glucose), EKGs, vision/hearing screens, and specimen collection/labeling with chain-of-custody as applicable.
  • Administer medications per scope and policy (e.g., PO, IM, SQ, inhaled, topical); monitor for adverse reactions and escalate concerns promptly.
  • Provide immunizations per standing orders; conduct wound care, dressing changes, ear irrigation, splinting, and other approved procedures.
  • Deliver patient education and whole health coaching using teach-back; reinforce self-management and care plans.
  • Participate in daily huddles, pre-visit planning, and post-visit follow-up to ensure continuity and access.
  • Support chronic disease and preventive care programs; perform outreach to close care gaps and reduce no-shows.
  • Coordinate referrals, imaging, and labs; assist with transitions of care and closed-loop test/result tracking.
  • Prepare exam rooms and equipment; maintain PAR levels, clean/stock rooms, and ensure crash cart and emergency supplies are checked per policy.
  • Assist providers with minor procedures; practice sterile/aseptic technique and instrument processing per policy (in collaboration with infection prevention).
  • Optimize flow via efficient in-rooming, task delegation, and template adherence to reduce cycle time and support same-day access.
  • Adhere to VA directives, nursing standards, and regulatory requirements (HIPAA/Privacy, OSHA, CLIA where applicable).
  • Follow infection prevention practices (hand hygiene, PPE, cleaning/ disinfection, temperature logs); report safety events and near misses.
  • Maintain competencies, BLS certification, and required annual trainings; participate in audits, tracers, and quality Improvement (PDSA) activities.
  • Document care in the VA EHR (e.g., CPRS/Cerner) accurately and timely; complete clinical reminders and standardized templates.
  • Enter orders per protocol when delegated and allowed by policy; ensure accurate specimen labels and test orders.
  • Protect Veteran privacy and information security at all times.
  • Completes required organizational compliance education, including assigned requirements that are client-specified, for Joint Commission Healthcare Staffing Services certification or other regulatory bodies.
