What you'll be doing:
- Developing long term case plans with residents and assisting them to implement strategies to achieve their goals by building on their strengths, aspirations and focusing on solutions.
- Initiating and maintaining contact with residents using flexible engagement strategies to build positive relationships that support self-determination, independence and achieve their case plan goals.
- Working closely with Unison Housing to support residents to address tenancy related issues to sustain tenancies.
- Advocating for residents in legal/housing matters through linkage to professional legal counsel and the provision of support documentation, liaison with the landlord, external mental health and legal service providers, and/or attendance at VCAT and or court.
- Advocating on behalf of residents with other agencies to ensure access and delivery of services.
- Facilitating client access to community resources and services (e.g. general health, mental health, housing, recreation etc) and work collaboratively with allied health services to achieve an inclusive and coordinated response to client needs.
- Facilitating and/or participating in case conferences as required and working proactively with residents and other supports to assist them to develop and maintain independent living skills.
What we're looking for (key selection criteria):
- A relevant tertiary qualification in human services, social work or other related disciplines.
- Experience in the delivery of support services to residents with complex needs relating to homelessness.
- Understanding of housing and homelessness policy, context and systems.
- Ability to build positive relationships and communicate with people of diverse backgrounds and abilities.
- Maintains high level of confidentiality.
- Ability to work collaboratively with others towards effective client solutions.
- Excellent interpersonal skills and communication skills, both verbal and written.
- Well-developed organisational and time management skills.
- Excellent problem-solving skills.
- Understanding of trauma-informed care and a recovery focused principles and practice
- A valid Victorian driver’s licence (mandatory).
