This model of care will operate seven days a week, providing responsive support to people in their last days of life. The aim is to help individuals spend time in the place they choose, ensuring dignity and comfort. You will work alongside District Nursing Teams and closely with other health and social care partners. This collaboration ensures a seamless experience for people in our care and their families and carers.
Key responsibilities:
- Deliver personalised, compassionate care to individuals at home
- Collaborate with multi-disciplinary teams to support seamless care
- Offer support to families and carers during times of need
- Contribute to shaping the future of care around dying in the Scottish Borders
What we’re looking for:
- Experience delivering care in a health or social care setting
- Full UK driving licence and access to your own vehicle
- NVQ Level 2 qualification in Health and Social Care (or equivalent), or a willingness to work towards this qualification
- A nurturing and compassionate approach, with excellent communication skills
- The ability to work independently while being an integral part of a multidisciplinary team
