Virtual Care Coordinator
Job Overview
The Virtual Care Coordinator will serve as the primary point of contact for patients managing Diabetes and Fatty Liver Disease, ensuring high-quality care coordination, education, and follow-up. This role involves working closely with physicians, nurse practitioners, dietitians, and other members of the healthcare team to improve patient outcomes through proactive communication, monitoring, and engagement.
Key Responsibilities:
- Serve as the liaison between patients and the care team to coordinate treatment plans, lab tests, follow-ups, and referrals.
- Provide education on disease management, lifestyle modifications, and medication adherence specific to Diabetes and Fatty Liver Disease.
- Conduct virtual check-ins with patients to track progress, address barriers, and escalate clinical concerns to providers when needed.
- Assist patients with appointment scheduling, lab/imaging reminders, and navigating healthcare systems.
- Monitor and document patient-reported outcomes (blood sugar logs, weight tracking, dietary changes, etc.).
- Identify high-risk patients and ensure timely interventions by coordinating with providers.
- Support patient enrollment in care programs, including chronic care management (CCM) and remote patient monitoring (RPM).
- Maintain accurate, HIPAA-compliant records in the electronic medical record (EMR).
- Educate patients on nutrition, exercise, and healthy habits to support liver health and glycemic control.
- Collaborate with insurance/billing teams to ensure program compliance and assist with care-related documentation.
Qualifications:
- Required:
- Experience in patient care coordination, nursing, medical assisting, case management, or related healthcare field.
- Strong knowledge of Diabetes management and awareness of Fatty Liver Disease (NAFLD/NASH).
- Excellent communication skills with the ability to explain medical concepts in plain language.
- Proficiency in telehealth platforms, EMR systems, and virtual communication tools.
- Strong organizational and multitasking skills.
- Preferred:
- Certification as a Medical Assistant, Licensed Practical Nurse (LPN), Registered Nurse (RN), or Certified Diabetes Care and Education Specialist (CDCES).
- Prior experience in chronic care management or remote patient monitoring.
- Bilingual language skills (depending on patient population).
