Job Overview
Salud’s Ambulatory Care Clinical Pharmacy Specialists (CPS) are the medication-use experts on Family Medicine patient care teams and provide comprehensive medication management. The CPS approaches his/her tasks in a team-based fashion that supports patients and their families in self-management, self-efficacy, and behavior change. Primary responsibilities of this position include participating in ambulatory care clinical practice at Salud Family Health Services. The CPS provides direct patient care within a multidisciplinary model, including care under collaborative drug therapy management protocols, population health services, and medication consultant services.

- Starting Annual Salary - $132,000 to $151,000
- Mon to Fri - 9 am to 5 pm

Specific Duties:
- Operates within the clinical scope and supervision of the clinical pharmacy Collaborative Drug Therapy Management Protocol, as well as all clinical pharmacy Standard Operating Procedures
- Engages in providing clinical care services in ambulatory care, both in person and through telehealth models
- Works in conjunction with Medical Providers, Dental providers, Behavioral Health Providers, and all other staff to ensure that patients obtain the highest standard of integrated health care
- Participates in organizational multidisciplinary committees and workgroups relating to medication use. Examples include quality teams, research projects, Peer Review, and Thrive's new Provider training
- Provides education to other team members on new medical guidelines, updates in therapy, etc.
- Serves as a regular medication resource/mentor for new primary care providers
- Documents consistently and appropriately for services completed in the electronic health record
- Performs comprehensive medication reviews to identify, prevent, and resolve medication-related problems as requested by primary care providers
- Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with their visit to Salud
- Instructs interns, students, and other medical personnel on matters pertaining to pharmacy
- Attends and participates in other activities conducted by Salud (i.e., staff training, educational sessions, meetings, etc.)
- Helps to ensure compliance with all federal and state rules and regulations
- Follows procedures for the electronic health records system
- Performs other duties as assigned
