Women’s Wellness Connection Patient Navigator
As a grantee of Colorado Department of Public Health and Environment (CDPHE), Salud is able to offer the Women’s Wellness Connection (WWC) Program to eligible patients for cancer prevention and early detection. The WWC Patient Navigator functions as a member of the patient care team, performing patient navigation for both insured and uninsured women through the Health Navigation and Clinical Services component of the WWC Program. The WWC Patient Navigator approaches his/her tasks in a team-based care fashion and provides support and guidance to patients throughout the breast and/or cervical cancer screening process.
We offer:
- Starting Wage - $18.93 to $21.77 per hour
- Monday to Friday – 8 am to 5 pm or 7 am to 4 pm

Specific Duties:
- Works in conjunction with Medical Providers, Medical Assistants, Enrollment Specialists, Customer Service Associates, and all other staff to ensure that patients obtain the highest standard of integrated health care
- Works with external sub-contractor facilities to ensure that patients are scheduled and followed up on
- Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with the breast/cervical cancer screening process
- Provide resources to reduce barriers, refer patients to appropriate resources
- Utilizes reports on potentially eligible women to perform outreach and schedule screening appointments
- Ensures all CDPHE and Salud paperwork has been completed accurately
- Review all patient application documents to ensure completion of all applicable documents and that eligibility requirements are met prior to approving the patients for the program
- Tracks patients enrolled into the WWC program to ensure follow-up imaging is completed and that results are communicated in a timely manner
- Enters data into eCaST (CDPHE’s database)
- Adheres to all standards, protocols, and laws surrounding patient confidentiality and compliance of program requirements
- Follows procedures for the electronic health records system
- Performs other duties as assigned
