The Relief Social Work Resource Coordinator serves as an advocate and point of contact for assigned patients and families through assisting the Licensed Clinical Social Worker or Social Work Clinician with carrying out the psycho social treatment plan. The Relief Social Work Resource Coordinator is responsible for assisting patients and families with resources throughout treatment at Lucile Packard Children's Hospital. The goal of this position is to ensure access to appropriate resources, improve patient experience and to help resolve issues around complicated resource needs that impede the patient and family receiving optimal care. The Relief Social Work Resource Coordinator is responsible for performing delegated tasks as assigned by the Social Workers with whom they are working such as coordinating resources and referrals to community assistance programs. Must perform all duties and responsibilities in accordance with the hospital's policies and procedures, including its Service Standards and its Code of Conduct.

* Collaborates with Social Workers, and other key people involved in the daily activities of providing medical care to patients at LPCH. Will attend interdisciplinary rounds, care conferences and other meetings as appropriate regarding the plan of care for the patient.
* Conducts assessments and makes referrals to appropriate resources in collaboration with the Social Work team.
* Will act as a primary liaison between LPCH and community resource agencies.
* Will assist in maintaining a current resource database for the Social Work Department for families.
* Coordinates transportation, lodging, meals or other needed resources and will maintain a database for monthly accounting for distributed assistance and reimbursement submitted.
* Participates in the development and implementation of new programs, operating protocols and communication initiatives related to resource coordination for families.
* Serves as the liaison between the clinical staff and family. Communicates needs for education, referrals and resources to appropriate direct care providers. Generates new resources and referral options for patients and families.
