What You’ll Do 

 

Position Summary

The Community Health Worker (CHW) will primarily focus on Prescription Assistance Support to ensure patients who cannot afford prescription medications have access to needed therapies. This includes conducting eligibility assessments, assisting patients with Low-Income Subsidy (LIS) and Patient Assistance Program (PAP) applications, gathering documentation, completing forms with patients, and following through until approval or referral to alternate resources. This temporary but high-impact role is especially critical during annual PAP renewal seasons or when there are high volumes of new patient enrollment. 

 

In addition, the CHW supports broader clinic care coordination and social needs navigation in collaboration with the multidisciplinary care team. This individual establishes trusting, supportive relationships with patients and families, helping them meet their medical and social needs while improving outcomes through access to resource

 

Responsibilities

  • Assess patients for eligibility for LIS, PAP, and other prescription assistance programs 
  • Help patients understand program requirements, gather necessary documentation, and complete applications. 
  • Submit and track applications, troubleshoot barriers, and provide consistent follow-up to ensure approval or timely redirection to alternative assistance. 
  • Maintain accurate records in the EMR and internal tracking spreadsheets for all medication assistance activities. 
  • Communicate effectively with prescribing providers and patients to ensure continuity of care and timely medication access. 
  • Coordinate with multiple Suvida clinic sites to ensure all clinics meet their annual PAP renewal goals and medication assistance targets. 
  • Facilitate clear and timely communication between clinics, providers, and support staff to promote best practices and consistency in application completion and follow-up. 
  • Organize workflows and track renewal progress across clinics to identify and address potential gaps or delays. 
  • Use NeedyMeds and other reliable resources to search for manufacturer prescription assistance programs tailored to the patient’s medications. 
  • Review and interpret eligibility criteria from NeedyMeds and manufacturer websites to determine patient qualification for assistance programs. 

 

General CHW Functions (Secondary/As Needed) 

  • Provide care coordination support to an assigned caseload of patients. 
  • Collaborate with patients, caregivers, providers, and community agencies to address complex social needs. 
  • Assist with referrals, benefits navigation, social service program education, and closed-loop follow-up for community resources and government programs. 
  • Participate in clinic operations such as patient onboarding, answering phones, and tracking patient engagement in care programs. 
  • Document all interactions in the EMR with accuracy and timeliness. 

 

What You’ll Bring

 

Knowledge, Skills, and Abilities

  • Prior experience assisting patients with LIS, PAP, or other benefits applications strongly preferred. 
  • 4–5 years of healthcare or social service experience with emphasis on care coordination or resource navigation. 
  • Skilled in EMR use, Microsoft Office/Google Suite, and spreadsheet tracking. 
  • Demonstrated experience or skills in coordinating activities across multiple sites or teams, including communication, workflow organization, and goal tracking. 
  • Experience managing or supporting performance goals across multiple clinical locations is a strong asset. 
  • Bilingual (English/Spanish) required. 
  • Strong organizational skills with the ability to manage high volume during peak renewal seasons. 
  • Compassionate communication and ability to work effectively across cultures. 
  • Skilled in EMR use, Microsoft Office/Google Suite, and spreadsheet tracking.

  • Bachelor's degree (preferred)
