Shifts: Within 8.00am - 4.00pm on Weekdays and Weekends

Duties

  • Co-lead the modernization and rollout of the HELP program across acute care units.
  • Develop workflows, tools, education, and volunteer training to support delirium prevention and functional preservation for older adults.
  • Contribute to Older Adult Action Plan initiatives, clinical pathway redesign, and QI/evaluation work.
  • Serve as an expert resource on delirium prevention and elder-friendly care practices.
  • Provides assessment and treatment of acute care in-patients at the Toronto Western Hospital
  • Provides evidence-based and patient centered practice to patients seen
  • Supports the development and implementation of best practice models of occupational therapy in the acute care setting at TWH
  • Works with multidisciplinary teams
  • Participates in the clinical education of occupational therapy students
  • Participate in Allied Health meetings and quality initiatives
  • In cooperation with other members of the occupational therapy in-patient team, provides assessment and treatment for patients across the hospital as required.
