Job Summary

The Social Worker for UHN’s Blood Disorders Program will primarily support adult outpatients in the Red Blood Cell Disorders Clinic, including patients living with hemoglobinopathies such as Sickle Cell Disease and Thalassemia.

The successful candidate will work collaboratively with an interdisciplinary team to provide psychosocial assessment, counselling, advocacy, system navigation, resource coordination, and support for patients and families. This role requires a strong understanding of health equity, social justice, and the structural barriers that affect patients living with chronic and complex blood disorders.

Duties

• Collaborate with a core interdisciplinary team to support patients living with hemoglobinopathies, including Sickle Cell Disease and Thalassemia.
• Demonstrate an excellent understanding of oppression through an intersectional lens, including but not limited to discrimination, racism, Anti-Black racism, ableism, disablement, Islamophobia, xenophobia, and the impacts of poverty and social exclusion.
• Demonstrate experience working with diverse patient communities to address inequities, improve patient experience, and support improved health and psychosocial outcomes.
• Strong communication skills and experience applying social justice and health equity frameworks are essential.
• Assess patients to determine the need for service, identify psychosocial concerns, and formulate recommendations, treatment plans, or intervention strategies.
• Develop, implement, reassess, and modify intervention plans in collaboration with patients, family members, significant others, and the interprofessional team.
• Provide counselling, advocacy, crisis support, care coordination, and system navigation to support patients’ psychosocial functioning and overall care.
• Liaise with appropriate community agencies, social services, and health system partners.
• Complete applications for services and supports as necessary.
• Establish and maintain communication with members of the interprofessional team and community agencies to obtain, provide, or exchange information related to patients’ psychosocial functioning and care needs.
• Provide services that benefit patients, families, the hospital, and the broader community.
• Provide clinical teaching and supervision to social work students, medical students, professional colleagues at University Health Network, Faculties of Social Work, and community partners.
• Participate in relevant research, patient engagement, patient education, quality improvement initiatives, and program development activities.
