Social Worker - BSW - PT

About the role

The Social Worker’s scope of practice includes providing case management and comprehensive social services to patients, families, and community members in coordination with staff throughout the acute care hospital. This role is responsible for supporting patient needs, facilitating resources, and coordinating care to promote positive outcomes. The Social Worker will maintain professional, supportive, and collaborative relationships with patients, residents, families, and team members.

Qualifications

• BSW from a school approved by CSWE Preferred. RN considered.
• Case Management experience preferred, ability to work effectively with allied personnel. Ability to communicate with culturally diverse populations in non-judgmental and therapeutic manner. Knowledge of age-specific norms, abuse reporting and procedural issues. Preference for knowledge/licensure in the principles and practices of social work.

Starting wage based on experience, with credit given up to 10 years.
